32nd ACBICON - December 18th - 21st 2005 Dyslipidemia and atherosclerosis Prof. Md Suharb Ali
Lipids and lipoproteins have been known to be of profound research interest of the past few decades. The strong association between dyslipidaemia and development of atherosclerosis is well established. Positive correlation between high serum total cholesterol, high low–density lipoprotein-cholesterol, low high – density lipoprotein-cholesterol and high triglycerides with atherosclerosis is well accepted. Various factors influence lipid and lipoprotein metabolism thereby changing the lipid profile. Age obesity, social class education, smoking alcohol health knowledge, dietary habit and diseases like diabetes mellitus, hypertension, thyroid disorders are the factors well studied . Patients with diabetes mellitus have 2 to 4 fold increased risk of atherosclerosis. Hyperglycemia increases the formation of oxidized LDL, and glycated LDL, Which are important modulators of atherosclerosis. Several epidemiological studies have shown an association between levels of HDL-C, postprandial lipemia and small dense LDL-C particles and the occurrence of cardiovascular disease. The etiology of hyperlipidemia and atherosclerosis is complex and governed by multiple interacting genes. However mutations tn two genes have been shown to be directly involved, i,e - the LDL receptor (LDLR) gene and apolipoprotein E (ApoE)gene. Hypertriglyceridemmia associated with hyperinsulinemia, hyperleptinemia of the insulin resistance syndrome provide the microenvironment of expression if genes responsible for atherosclerosis. Mesurment of inflammatory markers may add to the predictive value of total and high – density lipoprotein cholesterol in assessing long term coronary risk. The role of oxidative stress atherosclerosis is now hypothesized in terms of carbonyl stress that includes ‘Either Advanced glycation or Lipoxidation End-Products( EAGLEs) Hyperhomocystinemia has synergistic negative effect on blood vessels in diabetics due to its underlying endothelil dysfunction . Lipid modified protein have also been shown to be directly involved in different steps of atherosclerosis. Recent studies have shown that a) Individuals having small dense LDL-C have three times greater risk of developing atherosclerosis when compared to individuals with normal size LDL-C concentration) Small dense LDL-C shows greater atherosclerosis when compared to individuals with normal size LDL-C concentration .c) Presence of small dense LDS-C is normally seen in patients having elevated ApoB levels in spite of having normal LDC-C values. In atherosclerosis any one or all of the following biochemical changes might occur: a) Elevated VLDL, b) Elevated LDL C) Both LDL & VLDL elevated)Decreased HDL,E) High ratio of LDL cholesterol (Which normally : HDL-C =2.5:1) English Studies Department of State University of Bangladesh organizes Literary Afternoon
